Insured – The name of the insured must match the name of the company on your application/license EXACTLY.
Certificate Holder – must be Pikes Peak Regional Building Department, 2880 International Circle, Colorado Springs, CO 80910
Policy Number – A policy number must be on the certificate. A certificate that indicates the policy is pending or temporary or a binder will result in the insurance obligation expiring again in 30 days. Once a policy number has been assigned, a new insurance certificate must be provided.
10-day Cancellation/Reduction Clause – There must be an endorsement on the insurance certificatethat states that notification will be provided if the policy is reduced or cancelled within 10 days.

Workers’ Compensation – A current certificate of Workers’ Compensation in compliance is required if the contractor has employees. If the business has no employees, a Rejection of Coverage in compliance with the Colorado Department of Labor and Employment Division of Workers’ Compensation is required.
